What to do in case of incident?
What have you insured? *
Claims Procedure - RCA:
There are two solutions for resolving the case:
- By calling the Police Authorities;
- Based on the amicable report form (when the damage does not exceed 15,000 MDL).
Accident Reporting:
- In the case of involving Police Authorities:
- Call the emergency number 112 to report the accident;
- Stay at the scene until the police arrive or follow their instructions if advised otherwise;
- Notify the Insurance Company via email at [email protected], or [email protected] within 48 hours of the accident.
- In the case of an amicable report:
- Take photos of the accident scene showing both vehicles involved, including license plates, debris, etc.;
- Fill out the Amicable Accident Report Form properly.
- Notify the Insurance Company via email at [email protected], or [email protected] within 48 hours of the accident.
Documents Required for Submission by Persons Involved in a Traffic Accident:
- A copy of the police report and documents or, if applicable, the Amicable Accident Report Form;
- Technical inspection report (valid at the date of the incident);
- Copies of the RCA insurance policy, driver’s license, and vehicle registration certificate;
- Other documents requested by the Insurer, if necessary;
- Copy of the power of attorney, if applicable.

Claims Procedure - CASCO:
Call the emergency number 112 to report the accident.
Stay at the accident scene until the police arrive.
Notify the Insurance Company by email at [email protected], or [email protected], where you will indicate the number of the insurance contract, the date and place of the road accident and attach a photo of the accident.
Required Documents:
- Copy of the police report and documents or, if applicable, the Amicable Accident Report Form.
- Technical inspection report (valid at the date of the accident).
- Copies of the CASCO insurance policy, driver’s license, and vehicle registration certificate.
- For legal entities: Present the logbook or registration certificate (mandate).
- Other documents requested by Moldasig S.A., if necessary.
If the accident was caused by another driver: Contact the insurance company of the liable party and provide the necessary documents.
Exceptions:
The insurer will not pay compensation in certain situations, such as damage caused under the influence of alcohol or prohibited substances, leaving the scene of an accident or in the event of an expired roadworthiness test.
Health insurance claims procedure:
1. Accident insurance
To submit a reimbursement request, you need to visit the Claims Settlement Department of CA "MOLDASIG" SA at the address: mun. Chișinău, str. Petru Rareș 77, et. 3, office 304 or send the request via email at [email protected] or [email protected], attaching the following documents:
- Original Accident Insurance Policy
- Original ID card or birth certificate
- Copy of the medical report indicating the diagnosis.
- Copy of the medical leave certificate (if applicable).
- X-ray films (if applicable).
If the insured persons are abroad, documents can be completed, scanned, and sent via email to [email protected]
2. Voluntary Health Insurance (AFAM)
In case you have incurred expenses for medication purchases, dental services, surgeries, etc., to submit a reimbursement request, you need to visit the Claims Settlement Department of CA "MOLDASIG" SA at the address: mun. Chișinău, str. Petru Rareș 77, et. 3, office 304, or send the request via email at [email protected], or [email protected], attaching the following documents:
- Copy of the Medical Insurance Policy (AFAM)
- Copy of ID card or birth certificate (for minors)
- Copy of the medical recommendation (Referral Extract), results of consultations and investigations confirming the diagnosis, including receipts or invoices.
- Other documents requested, if applicable.
3. Voluntary Health Insurance (For Persons Traveling Temporarily Abroad)
In the event of an insured case abroad, the insured person must immediately contact the Assistance Company of the insurer using the telephone number or email address specified in the insurance policy and provide the following information:
- Full name
- Medical insurance policy
- Name of the medical institution in the country where the incident occurred
- Diagnosis established by the medical institution in the respective country
The Assistance Company will arrange for the provision of medical services, transportation, and other services specified in the insurance contract.
In case the insured person has incurred personal expenses for medical services abroad, to submit a reimbursement request, they need to visit the Claims Settlement Department of CA “MOLDASIG” SA at mun. Chișinău, str. Petru Rareș 77, et. 3, office 304, or send the request via email at [email protected] or [email protected], attaching the following documents:
- Copy of the medical insurance policy
- Copy of the passport or birth certificate (for minors)
- Copy of the medical recommendation (Referral Extract), results of consultations and investigations to establish the diagnosis
- Copies of receipts or invoices for services or medication purchases
- Other documents, as required
Voluntary Property Insurance Damage Procedure:
In the event of an insured case under the property insurance policy, you must visit the Claims Settlement Department of CA “MOLDASIG” SA at mun. Chișinău, str. Petru Rareș 77, et. 3, office 304, or notify us via email at [email protected], [email protected] or [email protected], with a brief description of the circumstances surrounding the case.
